Click on the "My Account" menu item in the left column and select the "Login" item:
Click on the “Forgot Login” link to see the next screen:
Click on the “Lost Password” checkbox.
Fill in your username and email address (must be the same email address as the one associated with your account) in the indicated text field, and click the “Send Password” button and you will receive an email with instructions to reset your password.
